Our experienced clinical staff partners with food service, activities, spiritual care and families to create personalized care plans for each resident. We are equipped to care for those with complex medical needs and chronic conditions, and all of our long-term nursing care facilities have spacious physical, occupational and speech therapy centers with modern equipment.
The majority of our long-term nursing care beds are in private rooms, making your loved one’s stay as comfortable as possible. However, we also offer semi-private accommodations, as we believe that having various options of room sizes and accommodations affords current and potential residents more choices, including lower-cost options.
We encourage residents and their families to take an active role in care planning. Our social services team is available to provide guidance, answer questions and help families stay informed throughout their loved one’s stay.

Payment for skilled nursing depends on the type of stay and individual circumstances:
- Medicare may cover short-term skilled nursing and rehabilitation following a qualifying hospital stay, for a limited period of time and while skilled care is medically necessary.
- Private pay and long-term care insurance may also be used for long-term skilled nursing, depending on individual coverage and benefits.
Concordia works with all prospective residents to help find funding and make things as affordable as possible.
