Comfort, Dignity and Compassionate Support When It Matters Most

Hospice care is specialized medical care for individuals facing advanced illness when the focus of treatment shifts from cure to comfort. Rather than pursuing aggressive curative measures, hospice emphasizes pain relief, symptom management and quality of life, helping patients live as peacefully and positively as possible.
At Concordia-IRMC VNA, hospice care supports the whole person – physically, emotionally and spiritually. Care is most often provided in the comfort of home, allowing individuals to remain surrounded by familiar settings and the people they love.
Hospice is not about giving up. It is about redefining hope by focusing on comfort, meaningful moments and dignity.

When is Hospice Care Appropriate?
Hospice care may be appropriate when:
- A physician determines a life expectancy of six months or less if the illness follows its expected course
- Curative treatments are no longer effective or are no longer desired
- Symptom management becomes the primary goal
- Hospitalizations are increasing or recovery is unlikely
- A patient and family wish to prioritize comfort and time together
Hospice services are provided through physician referral and may begin in a private residence, personal care home, assisted living community or other residential setting.
Our team works closely with our community providers to ensure thoughtful, coordinated transitions when hospice becomes the right next step.
