Long-term nursing care at Concordia at RP Home provides 24/7 skilled support for individuals who need ongoing medical and personal care. Residents are welcomed into a faith-based community where they receive the love, respect and kindness that we believe every person deserves.

Our experienced clinical team partners with food service, activities, spiritual care and families to create personalized care plans for each resident. We are equipped to care for those with complex medical needs and chronic conditions.
