Concordia Physician Practice provides primary and geriatric care in a convenient, faith-based setting on the Concordia at Cabot campus. Open to community members 18 and older, our practice offers compassionate, relationship-based care for individuals and families.

Dr. Sanjari completed his doctorate of osteopathic medicine at the Lake Erie College of Osteopathic Medicine at Seton Hill and his internal medicine residency at East Liverpool City Hospital. He previously served as a hospitalist at Heritage Valley Health System and brings broad experience in adult and senior medical care.
Dan McKnight, CRNP
Dan McKnight, CRNP is a Family Nurse Practitioner with clinical practice experience in internal medicine/adult geriatrics and a strong clinical background in emergency medicine. Working in medicine since 2016, his practice has been grounded in delivering comprehensive, patient-centered care for the adult patient, with a focus on accurate assessments, timely interventions and evidence-based treatments. His emergency medicine experience has equipped him with the ability to manage acute and complex conditions under pressure. His graduate degree is from Carlow University with a specialty in Family Practice. He and his family live in the Saxonburg area.
Stephanie Snow, CRNP
Stephanie Snow, CRNP has been a valued member of the Concordia Physician Practice since 2019. While her background is in critical care, Stephanie has found her passion in preventative and primary care for patients of all ages, helping them live healthier, longer lives. She is a board-certified nurse practitioner, earning her degree from Thomas Jefferson University in Philadelphia. Stephanie lives locally with her family and is committed to getting to know each patient as an individual and building long-term relationships within the community she calls home.
