Continuous Care (In-Home)

Continuous care involves the prolonged presence of a hospice nurse or aide in the patient’s home during crisis situations. It may be offered in the home when additional care is temporarily necessary to control particularly difficult symptoms, and when the family caregiver does not have the ability to manage the patient’s medical needs.
