In-Home Hospice Care

Hospice care is “comfort care” when a cure is no longer probable. In the patient’s home, it provides comfort through pain control and symptom management while offering emotional, social and spiritual support for the patient and family, individually and as a unit.

When it comes to hospice care in Washington County, we don’t believe you’ll find better, more compassionate caregivers than you’ll get with Concordia Hospice of Washington. While no two patients or cases are alike, In-Home Hospice Care often is as follows:

• A hospice patient typically has a registered nurse (RN) come to the home 1 – 3 times a week, depending on need. There is also an on-call RN for emergencies.
• RNs will come to the home to do assessments with the patient, including effectiveness of medications, new problems/concerns, and education for caregivers and patients.
• Home hospice aides will come to the home to perform and assist with activities of daily living. Frequency is based on need.
• A social worker is available for various reasons and will visit you at least monthly.
• A counselor will be available for various reasons and will visit you at least monthly.
• Volunteers are available as needed to provide various services, such as visitation, respite for caregivers, meal preparation, errands, light housekeeping, and telephone reassurance.
• A spiritual counselor is available as needed and if requested.
• An emergency medication kit will be kept in the home so there will be no delay in treatment when necessary.
• The patient keeps his/her own physician.

For those patients on Medicare, we can sometimes arrange for your loved one to stay a few days in our Donnell House hospice inpatient facility, which also gives you a reprieve from the stress of caregiving.
