November holds a special place in my heart as a season of gratitude—a time to reflect on and give thanks for the many blessings in our lives. For the Concordia home and community services family, this month carries even greater significance as we honor National Home Health Care, Home Care, Palliative and Hospice Care Month.
Twenty-seven years ago, we witnessed a meaningful shift in patient preferences—a desire for private duty and hospital-level care delivered in the comfort of home. Surrounded by personal belongings, familiar routines and the support of loved ones, home became the ideal setting for healing and care. In response to this growing need, Concordia launched its first home and community services agency, ConcordiaCare, in 1997.
Today, Concordia’s home and community services family includes two home care agencies, one durable medical equipment agency, four home health care agencies and four hospice agencies. Over the past fiscal year, our collective team served 29,028 persons in 573,125 visits, 6,279 new medical equipment orders were processed and direct care clinicians traveled nearly 5 million miles to deliver high-quality care to those we serve.
To our team – thank you for the countless blessings you bring to Concordia, to one another and to those we serve. The strength of our agencies continues to grow each year because of your compassion, respect, dedication to preserving dignity, professional achievements and unwavering commitment to patient-centered care. You carry our mission far beyond Concordia’s walls and each patient visit, inspiring us to strive for excellence every day. We are deeply grateful that you have chosen to be part of the Concordia family.
